Re-Registrations Sheets from the Previous Season – e.g. Players Carried Forward: a) Check that all the details held (shown) are correct. ADD Post Codes if not shown b) If a player has moved house – then strike out neatly the old information. Add the new address with POST CODE and telephone number/email address – if available. c) Player MUST sign in the box provided to verify the accuracy of information. d) Parent/Guardian MUST sign in the box provided to verify the accuracy of information. e) Supply new passport type photo – if player’s features have changed. Club reps must take responsibility for this and use common sense. f) Make sure that the player’s name is printed clearly on the back of the photograph. g) RETAIN FOR YOUR OWN RECORD PURPOSES A SPREADSHEET OF ALL DATA New Player Registrations (Triplicate Forms) a) Complete a triplicate Player Registration Form making sure that all address details including POST CODES and DOB’s are shown. b) Send in copy of player’s Birth Certificate – to prove EACH player’s age. c) Send in TWO passport type photographs – with names of EACH player printed on the reverse side. d) Ensure that all documents are fastened together very securely. e) Player MUST sign f) Club Representative MUST sign g) RETAIN FOR YOUR OWN RECORD PURPOSES A SPREADSHEET OF ALL DATA Send ALL documentation to my home address of: If multiple documents, you are urged to use registered post – and obtain from the Post Office a receipt. ALL DATA MUST BE CLEAR AND LEGIBLE The League is creating its own database of players – so please help me to help you – with the supply from time to time of accurate information.
